Educational Resources for Patients
Managing Your Triglycerides and Other Lipids
The American Heart Association (AHA) and the Nurse Practitioner Healthcare Foundation (NPHF) recognize the need for clinicians to educate their patients in the prevention, diagnosis, and treatment of elevated blood lipid levels. In order to address "best practice" treatment guidelines, promote patient adherence to treatment recommendations, and provide accurate educational information, the AHA and NPHF have developed a patient brochure: Managing Your Triglycerides and Other Lipids. This special report is customized to provide easy-to-understand patient information about triglycerides and other lipids. It also includes information about nurse practitioner practice. Practices are able to customize this brochure by adding their contact information on the brochure. The brochure may be used in the exam room, or placed in the office waiting room as a complimentary handout.
The 8 page color brochure includes:
- best practice treatment guidelines
- content on risk factors, lifestyle changes and medication adherence
- trusted resources for more information
Brochures will be shipped FREE to requesting NPs. To preview a copy, see the PDF.
All quantities have been distributed at this time. If you would like to order this brochure, please send your order in. We will fill it when we get the brochure reprinted. To order your supply of brochures, please download the order form, which is a fillable PDF. Please return to the NPHF by e-mail, fax, or post. The addresses and number are on the order form.
